Evidence-Led NDIS Navigation From Application to Plan Reassessment
The strength of an NDIS application or plan change request is not determined by the number of documents submitted. It is determined by whether the available evidence clearly explains the participant’s disability, functional impact and support requirements.
Caring Lab provides structured NDIS navigation and advocacy support to help participants and families understand what evidence may be relevant, identify gaps and coordinate information from appropriate professionals.
NDIS Access Applications
When applying to the NDIS, evidence may need to explain:
- The disability or impairment
- Treatment history where relevant
- Permanency
- Functional impact
- Assistance required in everyday life
- Existing formal and informal supports
- How the disability affects participation and independence
The NDIA describes supporting evidence as information from treating health professionals that can explain disability, treatment, daily-life impact and professional recommendations.
Plan Changes & Reassessments
Circumstances do not remain static.
A participant may experience:
- Functional deterioration
- New disability-related needs
- Change in living arrangements
- Breakdown of informal supports
- Transition from school
- Employment changes
- Increased mobility needs
- New behavioural risks
- Change in care arrangements
- Hospitalisation
- New assistive technology requirements
Where the change is longer-term and means a participant needs more, less or different NDIS supports, the NDIA may consider changing the plan. Evidence from professionals may be required.
